God met Jacob not at a shrine but on open ground, hours after Jacob had lied to his father and fled his brother's threat to kill him. No altar was built and no ritual performed before the vision came; Jacob simply stopped because the sun set, used a stone for a pillow, and slept. The ladder linking heaven and earth is Scripture's picture of a God who reaches the lowest point in a man's life uninvited, before that man does anything to deserve it.
